Guidance, access to resources, and accountability are key components of success. Case management is the core of how our shelter program operates to provide these tools. Case management is the link between residents and resources and is a highly individualized process in which residents work with a case manager in order to identify issues and obstacles and to develop a strategy for regaining wholeness and self-sufficiency. OHM families meet with their case managers on a weekly basis to evaluate progress. It is through case management that residents are referred to internal or external support programs designed to address specific needs and encourage ongoing progress.
